MS, daily life, and the need for tailored support

Living with multiple sclerosis can challenge how a person moves, thinks, works, and cares for themselves each day. Symptoms may make even simple tasks feel exhausting, painful, or potentially unsafe. People living with MS often need practical, tailored support to keep doing the activities that matter most to them, coined “occupations.” Desired occupations range from dressing independently and preparing meals to returning to work, caring for family, or pursuing hobbies.

Occupational therapists are rehabilitation specialists who bridge the gap between the person, environment, and activity demands constituted by the “occupation.” After assessing the person-environment-occupation fit, the 
OT modifies each factor into a tailored individualized approach coined “intervention.”

Interventions encompass combinations of strategies and tools to make daily life simpler, safer, and more fulfilling. Skills training focuses on practicing the task itself, building the person’s abilities (strength, coordination, attention, and sequencing) through graded, real-world practice and feedback.Compensatory strategies change how the task is done to bypass limitations, including discovering tools and technology to simplify complex tasks to reduce the cognitive or physical load. Environmental adaptations modify the surroundings or tools to make 
activities safer and easier. Patient education teaches symptom awareness, pacing, self-management, and correct use of equipment.

Who OT is for and how to access services

Occupational therapy is appropriate for any individual experiencing difficulty maintaining functional independence. Because MS can affect nearly every aspect of function, from fine motor skills to memory and endurance, OTs are prepared to address a broad range of challenges in both newly diagnosed and advanced cases. People with MS can access OT through a neurologist, primary care provider, or MS clinic, and are often covered by insurance.

Because MS is a neurological condition with diverse symptoms, clinician expertise often overlaps with adjacent specialty credentials. For example, stroke and physical rehabilitation specialists address motor issues such as 
spasticity and weakness. Brain injury and cognitive training specialists are well-suited for cognitive and some vision-related changes. Low vision professionals can be relevant for optic neuritis and involved in visual changes.

Understanding what these credentials signify helps you choose an OT whose scope best matches your needs. As with any rehabilitation service, the most effective approach is to begin with clear personal goals and select a provider whose training and experience directly support those goals.

Cognitive changes: Memory attention, and mental fatigue

Certified brain injury specialists and certified cognitive assessment and training professionals evaluate attention, memory, organization, and processing speed. Interventions often blend organizational skills, task sequencing, problem-solving, and decision-making to support cognitive retraining. Intervention is often embedded in real activities such as daily scheduling, medication setup, and grocery shopping. Therapists may introduce strategies to reduce cognitive load and make tasks more manageable. Overall, these practitioners focus on supporting working-memory and helping patients participate more confidently in daily life.

Mobility and physical endurance

Certified stroke rehabilitation specialists or holders of the board certification in physical rehabilitation or gerontology apply neurological movement and endurance principles to help individuals living with MS maintain strength, balance, and mobility. They offer skills training for safe transfers, mobility, and fall recovery. 
Environmental adaptations might involve selecting and fitting mobility aids (canes, rollators, and wheelchairs), adjusting seat and surface heights, and modifying tasks based on the person’s abilities. Throughout the session, therapists monitor symptom triggers and match interventions to the person’s needs and capabilities to ensure both tolerance and safety. The larger goal is fall prevention and confidence.

Seating and mobility / assistive technology

Seating and mobility specialists and assistive technology professionals focus on optimizing the fit and positioning of mobility aids, so movement is energy-efficient, comfortable, and safe. Sessions often involve fine-tuning equipment to improve posture and reduce the risk of secondary complications, such as pain or 
pressure injuries. Training covers effective use of wheelchairs, scooters, and walkers. This includes efficient wheelchair propulsion, safe transfers, and the selection and setup of power-assist add-ons or alternate drive modes matched to terrain demands (speed, torque, slope). Clinicians may recommend and configure customized seating systems (cushions, backs, tilt or recline), custom controls, and walker accessories tailored to the person’s tasks and environments.

Extremity function

Certified hand therapists offer focused rehabilitation to restore arm and hand weakness, spasticity, and tremor. Intervention may include skills training in grasp-release, in-hand manipulation, and task-specific practice. 
Therapists may also target hand-eye coordination and body awareness – especially important when  sensation is reduced. Compensatory strategies can emphasize weighted or large-grip utensils, button hooks, 
stabilization techniques, and task simplification to reduce effort and tremor impact. Adaptation often involves custom orthoses (resting, antispasticity, and functional) and ergonomic set-ups with strategic positioning and stretching to manage tone and pain. For clients with swelling or circulation issues related to reduced activity or medication side effects, certified lymphedema therapists provide targeted care to reduce edema, restore range of motion, and improve comfort while protecting skin integrity.

Low vision

Optic neuritis and other MS-related vision changes (blurriness, diplopia, glare sensitivity, and field loss) can compromise safety and independence at home and in the community. Specialists in low vision and certified low vision therapists typically complete a functional vision evaluation, then tailor intervention to the person’s needs. Therapists may train weakened eye muscles to improve visual scanning and tracking. They might teach task-specific strategies to navigate complex activities to improve access to desired tasks. When vision changes intersect with sensory loss or impaired temperature discrimination, therapists also teach protective techniques to further reduce risk during daily tasks.

Sensory integration and coordination

Sensory dysfunction and coordination management practitioners may treat numbness, tingling, altered touch, and coordination problems with graded coordination drills and sensory re-education, often woven into real-life activities (e.g., fastening clothing, identifying textures). The goal is to retrain perception and refine motor control to reduce errors and effort. Adaptive strategies can include textured grips and weighted tools that provide clearer tactile feedback when sensation and temperature discrimination is unreliable. 

Environmental modification and aging-in-place

Maintaining safety and independence at home is essential for many people with MS, especially when balance, endurance, or spasticity fluctuate. Occupational therapists with a specialty certification in environmental modification or as certified aging-in-place specialists often deliver care in the home, bringing recommendations 
directly into the person’s environment to improve accessibility and reduce fall risk. They conduct comprehensive home-safety assessments (e.g. reviewing room layouts, identifying hazards, and accounting for real-world constraints) to create a highly personalized plan. Interventions may include recommending and training clients to use adaptive equipment (e.g., grab bars, shower seating, and handheld showers); repositioning frequently used items for better access; and practicing priority tasks such as toilet and shower transfers, or safer bed mobility.

These clinicians are skilled with low-tech solutions, such as nonslip surfaces or grab bars, and can also scale to larger projects – such as ramps, stairlifts, ceiling lifts, and smart-home controls. They prioritize the highest-yield 
modifications first to maximize safety and independence.

Assistive technology for daily living

Assistive technology professionals match clients with low- and high-tech solutions, ranging from ergonomic keyboards and pointing devices to voice assistants and screen readers and magnifiers. They are specialized in 
identifying tools to complement a client's goals, routines, and environments; ensure the chosen device fits the individual’s needs; provide proper training for safe use; and prevent abandonment through follow up support. Interventions may include skills training in voice commands or addition of compensatory strategies to reduce cognitive and physical demands. 

Because occupational therapy spans everything from cognition and vision to mobility, seating, home modification, and assistive technology, it’s important to recognize the wide scope of OT and then match it to what you need right now. Start by clarifying your goals (e.g., fewer falls in the bathroom, easier meal prep, staying on top of meds, or reading with less eye strain), then choose a practitioner whose training and certifications align with those goals. Selecting the right OT for your priorities ensures every visit focuses on targeted skills training, smart compensations, practical adaptations, and education that are realistic, so your energy is protected and your independence grows in the areas that matter most to you.
